17 Qualifying Conditions

New Jersey recognizes 17 qualifying conditions for medical cannabis. A registered healthcare provider must certify that you have one of the following:

  • Amyotrophic lateral sclerosis (ALS)
  • Anxiety
  • Cancer
  • Chronic pain
  • Dysmenorrhea
  • Epilepsy
  • Glaucoma
  • HIV/AIDS
  • Inflammatory bowel disease
  • Intractable skeletal spasticity
  • Migraine
  • Multiple sclerosis
  • Muscular dystrophy
  • Opioid use disorder
  • Post-traumatic stress disorder (PTSD)
  • Seizure disorder
  • Tourette syndrome

Additionally, any terminal illness with a prognosis of 12 months or less qualifies. Providers may also certify patients for conditions not on the list if they believe cannabis therapy is warranted, though this is at the provider's discretion.

Patient Card: Costs & Renewal

Card Fees

  • Digital patient card: FREE
  • Physical patient card: $10 (valid for 2 years)

The digital card is accessible through the MCP patient portal and is accepted at all New Jersey dispensaries.

Renewal Process

  • Card duration: 2 years from date of issuance
  • Physician certification: Must be renewed annually — your registered provider must reaffirm your qualifying condition each year, even though the card itself lasts two years
  • Begin renewal early: Start the process at least 30 days before your card or certification expires to avoid gaps in access
  • Renew online: Log into the MCP portal to update your certification and renew your card

Finding a Registered Provider

Only healthcare providers who are registered with the CRC can certify patients for the medical cannabis program. The CRC maintains a searchable directory:

CRC Provider Directory

With 1,530 registered providers statewide, patients have access to a growing network of physicians, nurse practitioners, and other qualified practitioners. Many providers offer telehealth consultations, making it easier to obtain or renew a certification without an in-person visit.

Why Keep a Medical Card? Key Benefits

With recreational cannabis available to anyone 21 and older, some patients wonder whether maintaining a medical card is worthwhile. For many, the answer is yes:

Tax Savings

Medical cannabis patients are exempt from the 6.625% state sales tax on cannabis purchases. They are also exempt from the Social Equity Excise Fee. For a patient spending $200/month, this saves approximately $160 per year in taxes — and for heavier users, considerably more. Over a two-year card cycle, the savings far exceed the cost of obtaining and maintaining the card.

Higher Purchase Limits

Medical patients can purchase up to 3 ounces per 30 days, compared to the recreational per-transaction limit of 1 ounce. This is particularly important for patients using high doses or multiple forms of cannabis.

Access for Ages 18–20

Recreational cannabis requires age 21+, but the medical program is available to qualifying patients age 18 and older. Minors may also qualify with parental consent and a caregiver designation.

Medical-Only Dispensaries

Five dispensaries in New Jersey serve medical patients exclusively, often providing a more personalized and patient-focused experience with shorter wait times and specialized product selections.

Stronger Legal Protections

The Jake Honig Act provides specific workplace protections for registered patients, including protections against adverse employment actions based solely on their status as a medical cannabis patient.
