Before You Begin: Eligibility
To apply for a New Jersey medical cannabis card you must:
- Be a New Jersey resident with a valid state-issued ID or driver’s license. Out-of-state patients cannot apply for an NJ card — see reciprocity.
- Be at least 18 years old. Minors may be registered with a parent/guardian as caregiver.
- Have a diagnosis matching one of New Jersey’s 17 qualifying conditions.
- Not hold a commercial driver’s license (CDL). Federal regulations prohibit CDL holders from using cannabis under any circumstances, regardless of state law.
Step 1: Make Sure You Have a Valid New Jersey ID
New Jersey residency is verified using a state-issued driver’s license or non-driver ID. If you have recently moved to New Jersey, update your ID with the NJ MVC before applying. The portal will reject applications submitted with out-of-state ID.
Step 2: Get a CRC-Registered Physician Certification
A CRC-registered physician, advanced practice nurse, or physician assistant must evaluate your medical history and, if appropriate, submit a certification electronically through the NJMMP system stating you have a qualifying condition. The physician submits the certification directly — you do not upload it yourself.
Telehealth Is Accepted
New Jersey permits telehealth evaluations for new patients and renewals. Several online platforms connect NJ residents with CRC-registered providers who can complete the visit remotely:
- NuggMD — typically $99–$149
- Leafwell — typically $129–$199
- Heally — typically $89–$150
Most telehealth visits last 10–20 minutes and can be done from your phone or computer. If a provider does not approve your certification, most platforms refund the fee.
What to Bring to the Visit
- A list of your current medications
- Records or documentation of your qualifying diagnosis (specialist notes, imaging, lab results, VA records, etc.)
- Your New Jersey ID
- A list of treatments you have already tried
Step 3: Register on the NJMMP Patient Portal
Once your physician submits your certification, you create an account at the official NJMMP patient portal. During registration you will need to upload or provide:
- A copy of your New Jersey driver’s license or state ID
- Proof of NJ residency (utility bill, lease, etc., if requested)
- A passport-style photo
- Your Social Security number (used for the state database)
- Caregiver information (required for minors; optional for adults)
Step 4: Confirm the Card Type and Fee
New Jersey is one of the most patient-friendly states on card cost:
- Digital card: FREE — download from the portal after approval.
- Physical card: $10 — optional; mailed to your registered address.
- Veterans, seniors (65+), SSI/SSD/Medicaid recipients: free physical card with documentation.
The free digital card removes the biggest cost barrier seen in most other state programs. The card fee is separate from the physician evaluation fee discussed in Step 2. Step 5: Receive Your Card
After your application is reviewed and approved by the CRC:
- Digital card: Available to download immediately from the NJMMP portal once your application is approved. The digital card is valid at all licensed New Jersey dispensaries.
- Physical card (if ordered): Mailed to your registered address within 7–14 business days.
You can begin purchasing as soon as you have the digital card; you do not need to wait for the mailed card.
Patients can register and manage their medical cannabis card online through the New Jersey Medicinal Marijuana Program patient portal. The digital registry identification card is available at no cost.

Common Reasons for Application Denial or Delay
- Physician not CRC-registered. Only providers registered with the CRC medical cannabis program may certify.
- Out-of-state residency. The applicant must be a New Jersey resident with an NJ ID.
- Photo problems. The portal requires a passport-style photo; sunglasses, hats, and side-angle photos are rejected.
- Mismatched information. The name on the certification, the ID, and the application must match.
- Diagnosis not on the list. Review the 17 qualifying conditions.
Special Cases
Patients Under 18
A custodial parent or legal guardian must register as the patient’s designated caregiver. The caregiver acquires, possesses, and administers cannabis on behalf of the minor. Both the minor and the caregiver are registered through the portal.
Caregivers for Adult Patients
An adult patient may designate up to two caregivers. Caregivers must be 18+, NJ residents, pass a background check, and register through the portal.
CDL Holders
Federal regulations governing commercial motor vehicles prohibit CDL holders from using cannabis. The New Jersey program does not issue cards to CDL holders.
